Questionnaire Template:

For those of you who want to fill out the personal questionnaire, but did not receive one, here is the template.

Instructions:

1. Highlight the text below the dashed line.
• (For those of you new to computers) You can do this by placing the cursor arrow in front of the "N" in the "Name:" below and clicking the left mouse button (on a PC or the only button on a Macintosh). Without letting up on the button, drag the cursor to the bottom of the page so all the questions are highlighted.
2. Press down on the control (Ctrl) key and while holding it down press the "C" key.

If you are using a Macintosh use the key with the Apple symbol instead of the control key.
3. Let up on both keys

Once you have the email window open, you paste the questionnaire questions into it by:

1. Make sure the cursor arrow or line is in the message box (not the "Subject" box)
2. Press down on the control (Ctrl) key and while holding it down press the "V" key.

Once again, Macintosh users use the key with the Apple symbol instead of the control key.
3. The questions should all appear in the email message space.
4. Go through the questions and type in your answers.
5. Do you have an electronic photo of yourself? Attach it and I will add it to your page.
6. Click on the send button and I should get the email.
